A number of conditions may be confused with hematospermia (hemospermia). These can often be identified by a careful history, physical examination, urinalysis and semen examination.
Conditions that can cause pseudohematospermia:
(1) hematuria
(2) blood from sexual partner
(3) melanospermia (from malignant melanoma metastatic to the prostate or epididymis)
(4) red or orange colored urine
(4a) secondary to drugs or chemicals
(4b) secondary to food
(4c) myoglobinuria
(4d) hemoglobinuria
(4e) porphyria
