The wastage of a medication can be a costly situation.
Situation: medication unable to be used and must be discarded
Causes for drug wastage:
(1) expiration (outdating)
(2) cancellation of an order after dose prepared
(3) administration failure with inability to deliver dose
(4) dispensed but not used (as in anesthesia)
(5) storage problem (exposure to heat, thawed, exposed to light, exposure to moisture, contamination of multi-use vial)
(6) degradation of product (precipitate, discoloration)
(7) inability to identify (unlabeled)
(8) oversized vials
(9) noncompliance with a dispensed medication
