Description

The National Kidney Foundation (NKF) identified a number of conditions that can cause an acute decline in the glomerular filtration rate (GFR) in a patient with chronic renal failure. The occurrence of one or more of these conditions should alert the clinician to the need for closer monitoring and more aggressive management to reverse the decline.

Conditions associated with a decline in the glomerular filtration rate (GFR):

(1) obstruction in the urinary tract

(2) dehydration and other causes of volume depletion

Drugs and chemicals associated with a decline in GFR:

(1) cyclosporine

(2) tacrolimus

(3) angiotensin-converting enzyme inhibitors

(4) angiotensin-2 receptor blockers

(5) COX-2 inhibitors and other nonsteroidal anti-inflammatory drugs (NSAID)

(6) aminoglycosides

(7) amphotericin B
