Description

The fruit of the Cassava plant (Manihot esculenta) is a major foodstuff in tropical Africa. The Cassava plant contains linamarin, a cyanogenic glucoside. If a person eats improperly prepared products, cassavism (locally termed konzo or mantakassa) may develop, which features toxic ataxic neuropathy.

Criteria for toxic ataxic neuropathy (TAN): 2 or more of the following

(1) bilateral primary optic atrophy

(2) bilateral perceptive deafness

(3) myelopathy

(4) peripheral neuropathy

Clinical presentation:

(1) difficulty walking secondary to weakened muscles in the lower extremities

(2) subsequent symmetrical spastic paraparesis with increased muscle tone and hyperreflexia

(3) bilateral extensor plantar responses may be elicited

The damage is often irreversible and can only be prevented by proper food preparation.
