Description

Spirito et al identified risk factors for infective endocarditis in a patient with hypertrophic cardiomyopathy (HCM). A patient at risk may benefit from antibiotic prophylaxis before procedures associated with a high risk of endocarditis. The authors are from multiple hospitals in Italy.


 

In a population with HCM, infective endocarditis occurs with an incidence of 1.4 per 1,000 person years.

 

Risk factors for infective endocarditis:

(1) presence of outflow obstruction (odds ratio 3.8)

(2) left atrial dilatation with diameter >= 50 mm (odds ratio 5.4)

 

Criteria for outflow obstruction - one or both of the following::

(1) maximum Doppler gradient >= 30 mm Hg (between left ventricle and aorta)

(2) prolonged mitral-septal contact during systole on M-mode echocardiography
