Hypertrophic Cardiomyopathy (HCM) | Factors for Infective Endocarditis

Purpose: To identify a patient with hypertrophic cardiomyopathy (HCM) who is at increased risk for infective endocarditis based on the study of Spirito et al.

Description

Spirito et al identified risk factors for infective endocarditis in a patient with hypertrophic cardiomyopathy (HCM). A patient at risk may benefit from antibiotic prophylaxis before procedures associated with a high risk of endocarditis. The authors are from multiple hospitals in Italy.


 

In a population with HCM, infective endocarditis occurs with an incidence of 1.4 per 1,000 person years.

 

Risk factors for infective endocarditis:

(1) presence of outflow obstruction (odds ratio 3.8)

(2) left atrial dilatation with diameter >= 50 mm (odds ratio 5.4)

 

Criteria for outflow obstruction - one or both of the following::

(1) maximum Doppler gradient >= 30 mm Hg (between left ventricle and aorta)

(2) prolonged mitral-septal contact during systole on M-mode echocardiography
