Description

Romond et al developed a cardiac risk score for patients receiving adjuvant chemotherapy for breast cancer. This can be used to estimate the risk of an adverse cardiac event within 5 years. The authros are from multiple hospitals in the United States participating in the National Surgical Adjuvant Breast and Bowel Project.

Patient selection: woman enrolled in the National Surgical Adjuvant Breast and Bowel Project trial B-31.

Outcome: cardiac event (symptomatic CHF, cardiac death), definite or probable

Parameters:

(1) age in years

(2) baseline percent left ventricular ejection fraction (LVEF)

CRS =

= ((0.04 * (age)) - (0.1 * (baseline LVEF percent)) + 7) / 4.76 * 100

The probability of a cardiac event increases as the CRS rises.

Score

Probability of Cardiac Event in 5 Years

< 48

< 2%

48 to 100

(0.00906 * ((CRS)^2)) - (0.9196 * (CRS)) + 25.77

> 100

> 25%

Performance:

• The area under the ROC curve was 0.72 for Romond et al.
