Chi et al reported a risk score for a patient with carbon monoxide poisoning, designated the FIRED score. This can help to identify a patient likely to have a poor outcome. The authors are from Chang Gung University and National Sun Yat-Sen University in Taiwan.
FIRED is an acronym for fire exposure, intentional, respiratory, every point for GCS and diabetes.
Patient selection: carbon monoxide poisoning
Outcome: poor if GCS < 13 at discharge or if the patient died
Parameters:
(1) diabetes mellitus
(2) Glasgow Coma Scale (GCS) on arrival
(3) intentional carbon monoxide poisoning
(4) exposed at a fire scene
(5) respiratory failure

Interpretation:
• minimum score: 0
• maximum score: 43
• A score >= 10 was associated with a high risk for a poor outcome.
Performance:
• The area under the ROC curve is 0.93.
