Description

Hu et al reported the CAPCT score for evaluating a patient following a pancreaticoduodenectomy. This can help to identify a patient at risk for surgical site infection. The authors are from Chongqing Medical University in China.

CAPCT is an acronym for the score components.

Patient selection: status post pancreaticoduodenectomy

Parameters:

(1) nature of the tumor (C)

(2) post-operative serum albumin concentration in g/L (A)

(3) pancreatic fistula (P)

(4) post-operative cough (C)

(5) peri-operative blood transfusion (T)

= SUM(points for all 5 parameters)

Interpretation:

minimum score: 0

maximum score: 8

The higher the score the greater the risk of a surgical site infection.

A score >= 5 is associated with a high risk for surgical site infection.

The score could be modified to have an intermediate risk category. The presence of a fistula and hypoalbuminemia (total score 3) would seem at some risk for an infection.

Performance:

The area under the ROC curve was 0.95 in the derivation and 0.77 in the test cohorts.
