Description

The brown tumor of bone is a reactive bone lesion seen in patients with hyperparathyroidism. It is a component of osteitis fibrosa cystica generalisata (Recklinghausen's disease of bone).


Patient selection: primary or secondary hyperparathyroidism

 

Common locations: long bones, pelvic girdle, clavicle, ribs, mandible

 

The patient may have multiple bone lesions.

 

Gross appearance:

(1) brown color (from hemosiderin)

(2) partially cystic, partially solid

 

Histologic appearance:

(1) numerous osteoclast-like multinucleated giant cells

(2) fibroblastic stroma that is hypervascular

(3) hemosiderin-laden macrophages

 

Differential diagnosis:

(1) other bone lesions with giant cells

 

Misdiagnosis can occur if the presence of hyperparathyroidism is not recognized.
