A number of benign disorders may be misdiagnosed as a well-differentiated liposarcoma. In turn, a well-differentiated liposarcoma may be misdiagnosed as one of these benign conditions.
Lipomas that may be mistaken for a well-differentiated liposarcoma:
(1) lipoma with fat necrosis (traumatized lipoma)
(2) lipoma with a central vacuole in its nucleus ("lockern", from the German)
(3) diffuse lipomatosis
(4) pleomorphic or spindle cell lipoma
Lipomas mixed with other elements:
(1) myolipoma
(2) angiomyolipoma
(3) cellular angiolipoma
Other tumors:
(1) lipomatous hemangiopericytoma
Nontumorous reactions
(1) fatty atrophy
(2) silicone reaction
(3) injected oil
To avoid misdiagnosis it is essential:
(1) to sample a suspicious lesion adequately
(2) to get an adequate history
(3) to consult if in doubt
(4) to order appropriate molecular testing
