Description

Mauss et al developed a prognostic model for survival in patients who have had an acute myocardial infarction. They used parameters available at the bedside to stratify patients for outcome. The authors are from Goethe University in Frankfurt am Main.

Patient selection: acute myocardial infarction

Outcome event-free survival: Events were all-cause mortality and arrhythmic events (sudden death, resuscitated ventricular fibrillation, sustained ventricular tachycardia). Reinfarction or revascularization procedures were recorded but were not used as study end-points.

Parameters evaluated at the time of hospital discharge

(1) left ventricular ejection fraction LVEF) in percent

(2) heart rate in beats per minute

LVEF

Heart Rate

5 Year Event Free Survival

> 35%

< 75 beats per minute

93%

> 35%

>= 75 beats per minute

89%

<= 35%

< 75 beats per minute

78%

<= 35%

>= 75 beats per minute

68%

For the patients with both risk factors the events occurred within the first 2 years after discharge.

The risk increased with the heart rate, with a heart rate >= 100 beats per minute associated with a higher rate of adverse events.
