Description

Savoldi et al evaluated antibiotic resistance in Helicobacter pylori. Antibiotic resistance is common for Helicobacter pylori, but the susceptibility pattern varies with the global region.


Antibiotic resistance should be suspected when there is failure of antibiotic therapy without alternative explanation (noncompliance, etc).

 

Therapeutic History

Resistance

prior to start of first eradication therapy

primary

>= 1 unsuccessful eradication attempt

secondary

 

Resistance was considered high when present in >= 15% of isolates.

 

Antibiotics with a high prevalence of resistance:

(1) clarithromycin

(2) metronidazole

(3) levofloxacin

 

Antibiotics with low prevalence of resistance currently:

(1) amoxicillin

(2) tetracycline

 

Susceptibility testing should be considered:

(1) if starting an antibiotic that has a high prevalence of resistance in the global region

(2) if an eradication attempt has proven unsuccessful
