Description

Cellucci et al evaluated children with primary angiitis of the central nervous system. They identified risk factors associated with higher disease activity. The authors are from the Hospital for Sick Children and the University of Toronto.


 

Clinical features of primary angiitis in children:

(1) seizures

(2) cognitive dysfunction

(3) behavioral changes

(4) headaches

(5) motor deficits including hemiplegia

(6) sensory deficits

(7) speech deficits

(8) visual disturbances

(9) fever

 

Imaging studies:

(1) variable angiography (some positive, others negative).

(1a) Positive angiography is usually associated with involvement of medium and large vessels.

(1b) Negative angiography is associated with lymphocytic vascularitis of small vessels.

(2) abnormal MRI including ischemic lesions

 

Laboratory findings:

(1) elevated ESR

(2) elevated CRP

 

Risk factors for higher disease activity:

(1) negative angiography

(2) seizures
