Anemia and hypophosphatemia can be seen in:
(1) hemolytic anemia
(2) chronic liver disease
(3) iron replacement of iron deficiency anemia
(4) diabetic ketoacidosis
(5) malnutrition with refeeding syndrome or hyperalimentation
(6) chronic renal failure with dialysis or transplant
