Derkay et al developed a staging system for patients with recurrent respiratory papillomatosis (RPP) which is based on 2 scores - one clinical and one anatomical. The combined score can be used to evaluate the patient initially and to monitor the response to therapy.
Sites evaluated in the anatomical score:
(1) the lingual surface of the epiglottis
(2) the laryngeal surface of the epiglottis
(3) the right aryepiglottic fold
(4) the left aryepiglottic fold
(5) the right false vocal cord
(6) the left false vocal cord
(7) the right true vocal cord
(8) the left true vocal cord
(9) the right arytenoids
(10) the left arytenoids
(11) the anterior commissure
(12) the posterior commissure
(13) the subglottis
(14) the upper third of the trachea
(15) the middle third of the trachea
(16) the lower third of the trachea
(17) the right bronchi
(18) the left bronchi
(19) tracheotomy stoma (if present)
(20) nose
(21) palate
(22) pharynx
(23) esophagus
(24) lungs
(25) other

Interpretation:
• minimum anatomcial score: 0
• maximum anatomical score: 75 (if tracheotomy stoma present); 72 (if no tracheostomy)
• The higher the anatomical score the more extensive the disease.
