Description

Bennett et al reported alternate criteria for the diagnosis of fibromyalgia. This was based on the modified 2010 preliminary ACR criteria. The authors are from Oregon Health & Science University, Stony Brook University, University of Pittsburgh, Rowan University and Providence Arthritis Center (Portland).


Parameters:

(1) pain location inventory based on count of pain sites

(2) 10-item symptom impact questionnaire (SIQR) symptom score

 

Pain sites - 28 (4 midline, 12 bilateral):

(1) neck

(2) mid-upper back

(3) front of chest

(4) mid-lower back

(5) jaw, left and right

(6) shoulders, left and right

(7) arms, left and right

(8) hands, left and right

(9) wrists, left and right

(10) hips, left and right

(11) thighs, left and right

(12) knees, left and right

(13) ankles, left and right

(14) feet, left and right

(15) upper back, left and right

(16) lower back, left and right

 

10-item SIQR (symptom impact questionnaire) symptom score - VAS from 0 to 10 cm:

(1) pain (none to unbearable)

(2) energy (lots to none)

(3) stiffness (none to severe)

(4) sleep (awoke rested to very tired)

(5) depression (none to very)

(6) memory problems (good to very poor)

(7) anxiety (none to very)

(8) tenderness to touch (none to very)

(9) balance problems (none to severe)

(10) sensitivity to loud noises, bright lights, odors and cold (none to extreme)

 

pain location inventory = PLI =

= SUM(number of pain sites out of 28)

 

SIQR symptoms score =

= SUM(VAS scores for all 10 questions) / 2

 

Interpretation:

• minimum PLI and SIQR symptom score: 0

• maximum PLI: 28

• maximum SIQR symptoms core: 50

 

Criteria for a high likelihood for fibromyalgia:

(1) findings persistent for >= 3 months

(2) pain location inventory >= 17

(3) SIQR symptoms score >= 21
