Hongo et al reported an algorithm for evaluating a male with epididymitis. This can help to identify a patient at risk for severe infection. The authors are from Keio University, Aichi Medical University and Tokyo Medical Center.
Patient selection: epididymitis
Severe epididymitis - one or more of the following:
(1) sepsis
(2) abscess formation
(3) exacerbation of symptoms
Parameters:
(1) age in years
(2) diabetes mellitus
(3) fever (body temperature >= 38°C)
(4) serum CRP in mg/dL
(5) serum BUN in mg/dL
Low risk: age < 65 years AND nondiabetic
High risk: (age >= 65 years OR diabetes) AND (temperature >=38°C) AND (CRP >= 6 mg/dL OR BUN >= 19 mg/dL)
Intermediate risk: everyone else
