Description

Chen et al developed an algorithm for evaluating a patient with bilateral parotid enlargement. The authors are from New York University.


 

Patient selection: bilateral parotid enlargement

 

Parameters:

(1) duration - acute (<= 3 weeks) vs chronic (> 3 weeks)

(2) symptoms - painful vs painless

(3) pattern - nodular vs diffuse

 

There are 8 combinations of these parameters.

 

Acute, painful and nodular:

(1) abscess

 

Acute, painful and diffuse:

(1) mumps

(2) acute suppurative parotitis

(3) recurrent parotitis

(4) Wegener's granulomatosis

(5) pneumoparotitis

(6) iodide constrast-induced

(7) radioiodine-induced

 

Acute, painless and nodular:

(1) MALT lymphoma

 

Acute, painless and diffuse:

(1) Wegener's granulomatosis

(2) pneumoparotitis

(3) anesthesia mumps

(4) radiodine induced

(5) medication induced

 

Chronic, painful and nodular:

(1) tuberculosis

 

Chronic, painful and diffuse:

(1) tuberculosis

(2) Sjogren's syndrome

 

Chronic, painless and nodular:

(1) tuberculosis

(2) HIV-related lesions (diffuse infiltrative lymphocytosis syndrome, benign lymphoepithelial cyst)

(3) Warthin tumor

(4) oncocytoma

(5) polycystic parotid disease

 

Chronic, painless and diffuse:

(1) tuberculosis

(2) sialadenosis

(3) tuberculosis

(4) Kimura disease

(5) Sjogren's syndrome
