Description

Chow et al reported the Algorithm Examining the Risk of Massive Transfusion (ALERT) Scores for identifying a patient requiring massive transfusion. There are scores for prehospital and admission states. The authors are from George Washington University, University of Maryland and the University of Oklahoma.


Patient selection: traumatically injured patient prior to arrival at the trauma center

(1) prehospital systolic blood pressure (SBP) in mm Hg

(2) prehospital diastolic blood pressure (DBP) in mm Hg

(3) admission systolic blood pressure in mm Hg

(4) admission heart rate in beats per minute

(5) admission respiratory rate in breaths per minute

(6) admission temperature in °C

(6) admission Glasgow Coma Scale, from 3 to 15

Interpretation:

• minimum score: 0

• maximum score: 47

• The higher the score the greater the likelihood of massive transfusion.

• A score >= 12 had a sensitivity of 0.78 and specificity of 0.62.

• A score >= 15 had a sensitivity of 0.25 and specificity of 0.96, with 89% correctly classified.

• A score >= 20 was 100% specific for massive transfusion (yet correct classification is reported as 90%).

 

Performance:

• The area under the ROC curve is 0.91, although this seems unusual based on the sensitivities and specificities reported.
