Description

Hartwig et al used a simple questionnaire to help to decide if an adverse drug reaction was preventable. The authors are from the Ohio State University.


Questions:

(1) Was the drug causing the adverse drug reaction appropriate/indicated for the patient?

(2) Were the following appropriate for the patient:

(2a) dose

(2b) route

(2c) frequency of administration

(3) If the reaction was an allergic reaction, then was the allergy

(3a) documented in the EHR?

(3b) documented at the time of admission?

(4) Was testing (therapeutic drug monitoring, laboratory testing, etc) performed that could have predicted the reaction?
