Description

A trigger tool can be used to identify patients who may have an adverse drug event (ADE). It can also be used to prospectively to identify patients who may be at increased risk for developing an ADE. This can function as an early warning system.


 

Clinical event triggers may include:

(1) oversedation, confusion or lethargy

(2) unexplained hypotension

(3) falls

(4) rash

(5) abrupt discontinuation of a drug

(6) called code

(7) unplanned transfer (to hospital, to ICU)

(8) delay in discharge

 

where:

• Abrupt discontinuation of a drug may indicate that an adverse effect was noted. It may also precipitate a withdrawal/discontinuation reaction.
