Description

A trigger tool can be used to identify patients who may have an adverse drug event (ADE). This can function as an early warning system or as a patient safety indicator.


 

Antidote administrations event triggers may include:

(1) vitamin K in a patient receiving warfarin or coumadin

(2) protamine sulfate

(3) naloxone (Narcan)

(4) antiemetic

(5) antidiarrheal agent

(6) therapy for hyperkalemia - sodium polystyrene (Kayexelate), other

(7) diphenhydramine (Benadryl)

(8) flumazenil (benzodiazepine receptor antagonist)

(9) epinephrine or other therapy for anaphylaxis
