Traumatic injury to the adrenal gland can be graded using the Adrenal Organ Injury Scale,
Type of Injury |
Involvement |
Grade |
contusion |
|
I |
laceration |
involving only cortex (< 2 cm) |
II |
|
extending into medulla (>= 2 cm) |
III |
parenchymal destruction |
> 50% but subtotal |
IV |
|
total, including massive intraparenchymal hemorrhage |
V |
avulsion from blood supply |
|
V |
where:
• I am unsure what the meaning of the laceration measurement in centimeters is. The cortex is usually only a few millimeters thick, while the entire adrenal gland is only a couple centimeters in any one dimension.
• Parenchymal destruction < 50% is not listed as an option.
The final grade for bilateral injuries is increased by 1 up to grade V.
• I am assuming that grade IV can be increased to grade V but grade V is not increased to grade VI.
