Barriers which may prevent recognition of a healthcare provider who is a serial killer:

(1) refusal to consider the possibility

(2) refusal to listen to repeated complaints about the provider

(3) "coverup culture"

(4) failure to track unusual incidents

(5) failure to investigate unusual incidents adequately (perform autopsies, etc)

(6) failure to perform an adequate background check on new employees

(7) failure to followup on reports of problems given for its employees

(8) chaotic work environment (high turnover of staff, frequent changes in staff, etc)

(9) inactivity caused by a fear of litigation
