Description

An acute tear in the fascia cruris at its attachment to the Achilles tendon and its paratenon may present as acute pain (Achillodynia).


The fascia cruris is a layer of connective tissue that encloses structures in the posterior calf and that connects to the paratenon of the Achilles tendon.

 

Features of a tear in the fascia cruris:

(1) There is the sudden onset of pain in the region of the Achilles tendon.

(2) It may be preceded by a strenuous or unaccustomed activity.

(3) There may be a history of calf tightness prior to onset.

(4) There may be swelling and tenderness over the medial or lateral aspect of the Achilles tendon in its midportion.

(5) Ultrasound shows a hypoechoic area that extends along the anatomical plane of the fascia cruris.

(6) The affected leg may show overpronation.

(7) There has been exclusion of other diagnoses including paratenonitis.
