Description

Gajic et al developed a model for predicting an adverse outcome in patients with acute lung injury. This can help identify a patient who may require more aggressive management based on parameters measured soon after admission to the Intensive Care Unit (ICU). The authors are from the Second International Study of Mechanical Ventilation and ARDS-net Investigators from the United States and Europe.


Outcomes:

(1) death

(2) prolonged mechanical ventilation

 

Parameters:

(1) age in years

(2) oxygenation index (see 08.07.12) on day 3 in the ICU

(3) cardiovascular failure on day 3 (systolic blood pressure < 90 mm Hg and/or use of vasopressors)

= (0.03 * (age in years)) + (0.07 * (oxygenation index)) + (points for cardiovascular failure)

 

Interpretation:

• A cutoff at > 3.0 is more sensitive for predicting poor outcome..

• A cutoff at > 3.5 is more specific.
