Description

Chawla et al described a scoring index for casts in the urinary sediment associated with acute kidney injury (AKI). This can help to standardize reporting of urinalysis findings. The authors are from George Washington University Medical Center in Washington, D.C.

Casts of interest:

(1) granular cell

(2) epithelial cell

Parameters:

(1) frequency of granular and/or epithelial cell casts (may have a muddy brown appearance)

(2) percent of low power microscopic field (10x objective) with 1 or more casts

Frequency of Casts

Percent LPF with Casts

Grade

none

0

1

rare

0.01 to 10% (at least 1 cast on the slide)

2

moderate

10.01- 89%

3

sheets

>= 90%

4

where:

• The absence of casts would make more sense if it were called Grade 0.

• There is a observational gap between rare and moderate.
