Description

Actinomycosis may rarely involve the gallbladder or bile ducts.


 

Sites of involvement may include any combination of:

(1) gallbladder

(2) cholecystic duct

(3) common bile duct

 

Clinical features:

(1) right upper quadrant pain

(2) acute or chronic cholecystitis or cholangitis

 

Direct smear or histologic exam shows Gram-positive branching bacilli with or without sulfur granules.

 

Complications:

(1) extension of the infection into the liver

(2) fistula formation

(3) mass effect that may mimic a malignancy
