Description

An accessory scrotum is a rare congenital disorder in males. It apparently occurs when there is a disruption in the continuity of the developing labioscrotal swelling.

The accessory scrotum is most often identified in the perineum but can occur at other sites such as the distal penile shaft.

Features:

(1) The patient has a normal scrotum.

(2) There is a separate skin tag-like structure with a rugose skin surface.

(3) Histology of this structure shows subcutaneous smooth muscle.

(4) No testis is present within the structure.

A perineal accessory scrotum may have a variable association with a lipoma (lipomatous hamartoma).

In some patients the accessory scrotum is associated with congenital anomalies (imperforate anus, genitourinary tract, etc).
