The Japanese Society of Wound, Ostomy and Continence Maintenance reported the ABCD score for evaluating an ostomy site.


Patient selection: ostomy

 

Parameters:

(1) skin adjacent (A) to the ostomy opening

(2) skin in contact with the skin barrier (B) of the ostomy appliance

(3) skin circumscribing the area (C) where accessories are attached

(4) discoloration

Interpretation:

• minimum total score: 0

• maximum total score: 45

• The sites are also reported as Ax By Cz with lower case letters indicating points for the site.

• The higher the score the more severe the changes.
