Dobbs and Fleming developed a scoring system using symptoms, history and urine dipstick ("SHU") findings to estimate probability of a urinary tract infection for a patient seen in a primary care setting. The authors are from Birmingham in England.
Patient population groups:
(1) children 0 to 14 years
(2) nonpregnant women 15 to 49 years of age
(3) nonpregnant women 50 years of age or older
Pregnant women and males over the age of 14 are not included in the scoring system.
Symptoms:
(1) frequency
(2) nocturia
(3) dysuria
(4) urgency
(5) hematuria
(6) offensive smelling urine
(7) nausea
History:
(8) duration of symptoms in days
(9) history of previous urinary tract infection (UTI)
(10) history of previous intravenous pyelogram (IVP)
Dipstick urinalysis testing:
(11) protein
(12) blood
(13) nitrite

SHU score =
= SUM(points for all of the parameters)
Interpretation:
• A score of 0 corresponds to a probability of infection of 50%.
• The lowest score appears to be –19.
• The highest score can be > 20 but at this point the probability of infection is > 99.9%.

• ABS = absolute value
• percent probability = (odds) / ((odds) + 1) * 100%
• percent probability with odds against = 1 / ((odds for ABS(SHU)) + 1) * 100%
