Singer et al reported a 3-item scale for screening a patient for stroke. The authors are from J. W. Goethe University in Frankfurt am Main.
Patient selection: possible stroke
Parameters:
(1) level of consciousness
(2) gaze and head deviation
(3) motor function (hemiparesis)
Parameter
Finding
Points
disturbance of consciousness
none
0
mild to moderate
1
severe
2
gaze and head deviation
absent
incomplete gaze/head deviation
forced gaze/head deviation
hemiparesis
total score =
= SUM(points for all 3 parameters)
Interpretation:
• minimum score: 0
• maximum score: 6
• A score >= 4 predicts proximal vessel occlusion (T-segment or M1-segment of the middle cerebral artery).
Performance:
• A score >= 4 has a sensitivity of 67% and specificity of 92% for proximal vessel occlusion.
• The scale is strongly associated with the NIHSS.
