Kiriyama et al reported the updated Tokyo criteria for acute cholangitis.


Patient selection: acute cholangitis

 

Grades: I (mild), II (moderate) or III (severe)

 

Criteria for Grade III - one or more of the following:

(1) hypotension requiring vasopressor support (dopamine >= 5 µg/kg/min or norepiphrine any dose; cardiovascular dysfunction)

(2) disturbance of consciousness (neurological dysfunction)

(3) PaO2/FIO2 ratio < 300 (respiratory dysfunction)

(4) oliguria or serum creatinine > 2.0 mg/dL (renal dysfunction)

(5) INR > 1.5 (hepatic dysfunction

(6) platelet count < 100,000 per µL (hematologic dysfunction)

 

Criteria for Grade II - 2 or more of the following:

(1) WBC count < 4,000 or > 12,000 per µL

(2) temperature >= 39°C

(3) >= 75 years of age

(4) serum total bilirubin >= 5 mg/dL

(5) hypoalbuminemia (less than 0.7 times the LLN)

 

Criteria for Grade I: not Grade II or III
