(12) weight loss within the past 6 months > 10%

(13) chemotherapy and/or radiation therapy prior to surgery

(14) history of metastatic cancer

(15) severe renal failure or currently on dialysis

Each item is scored 0 if absent and 1 if present.

In the original 11-item mFI, 2 items were mutually exclusive, so the maximum score is 14.
