Tinnitus from excessive noise in the workplace can result in impairment. The following rating scale for tinnitus is described by Dr. Glorig and can be used to assess the severity of the tinnitus.
The questionnaire consists of 7 questions.
The first determines if the person has ringing or noises in the ears. If not the person stops, else the person answers the next 6 questions.
(1) constant vs intermittent
(2) interference with going to sleep
(3) change in quiet environment
(4) change when inactive
(5) what it sounds like
(6) bother caused
