Patient selection: pregnant woman with intrauterine pregnancy of uncertain viability

Imaging: transvaginal ultrasonography

Parameter

Suspicious for

Diagnostic for

crown-rump length and heartbeat

< 7 mm AND no heartbeat

>= 7 mm AND no hearbeat

mean gestational sac diameter and embryo

16-24 mm AND no embryo

>= 25 mm AND no embryo

gestational sac without yolk sac

absence of embryo with heartbeat 7-13 days after scan showing sac

absence of embryo with heartbeat >=14 days after scan showing sac

gestational sac with yolk sac

absence of embryo with heartbeat 7-10 days after scan showing sac

absence of embryo with heartbeat >= 11 days after scan showing sac

empty

NA

yolk sac diameter

> 7 mm (enlarged)

different between gestational sac diameter and crown-rump length

< 5 mm

NA

where:

• The amnion is adjacent to the yolk sac.

difference between gestational sac diameter and crown-rump length =

= (mean gestational sac diameter in mm) - (crown-rump length in mm)

If the findings are suspicious for a failed pregnancy then repeat ultrasonography in 7-10 days.
