Description

Odibo et al developed a score for predicting preterm delivery at < 32 weeks gestation in women who have had cervical cerclage. This can help identify a patient who may benefit from closer monitoring and more aggressive management. The authors are from the University of Pennsylvania and Thomas Jefferson University Hospital in Philadelphia.

Parameters:

(1) cervical length

(2) history of cervical cone biopsy

(3) timing for the cervical cerclage

Interpretation:

• minimum score: 0

• maximum score: 3

• A score >= 2 was predictive of preterm delivery.

Performance:

• Sensitivity 80%, specificity 96%, Youden index 76%.

• Negative predictive value 95%, accuracy 93%.
