Relapsing seronegative symmetric synovitis with pitting edema may be associated with a number of underlying conditions.
Synonym: RS3PE (where S3 = seronegative, symmetric, synovitis); remitting seronegative symmetric synovitis with pitting edema
Conditions which may be associated with the syndrome:
(1) malignant lymphoma
(2) myelodysplasia or leukemia
(3) cancer (colon, lung, other)
(4) inflammatory disorder (polymyalgia rheumatica, other)
(5) idiopathic (primary)
Clinical features:
(1) abrupt onset of arthritis and edema affecting:
(1a) metacarpophalangeal joints
(1b) proximal interphalangeal joints
(1c) wrists
(2) tends to affect the elderly, males more than females
(3) rheumatoid factor negative
(4) tends to follow a relatively benign course
Patients with a malignancy related RS3PE may show:
(1) poor response to corticosteroids and NSAIDS
(2) constitutional symptoms (fever, night sweats, weight loss)
