Description

Zhang et al identifie risk factors associated with readmission following bypass surgery on a lower extremity. These can help to identify a patient who may benefit from more aggressive management. The authors are from Beth Israel Deaconess Medical Center.

Patient selection: lower extremity bypass surgery

Outcome: 30-day readmission (planned or unplanned, related or unrelated)

Unplanned, related readmissions were 75% of total.

Reasons for unplanned, related readmission included:

(1) wound complication

(2) graft failure

(3) multiple complications

Risk factors for readmission (odds ratios 1.3 to 1.5):

(1) dependent in functional status

(2) dyspnea (pulmonary comorbidity)

(3) cardiac comorbidity

(4) dialysis dependence (chronic renal failure)

(5) obesity

(6) malnutrition

(7) surgery for critical limb ischemia (CLI)

Risk factor for readmission (odds ratio 8.0)

(1) return to the operating room on index admission
