Description

Schwaber et al identified risk factors for acquisition of carbapenem-resistant Klebsiella pneumoniae in a hospitalized patient. These can help to identify a patient who should be screened for carriage. The authors are from Tel Aviv Sourasky Medical Center.

Patient selection: hospitalized adult in a region with carbapenem-resistant Klebsiella pneumoniae

Parameters:

(1) functional status

(2) admission to intensive care unit (ICU)

(3) receipt of antibiotics

where:

• The odds ratio associated with fluroquinolone therapy was 7.2 with risk from overall antibiotic exposure was 4.4.

number of risk factors =

= SUM(points for all 3 parameters)

Interpretation:

• minimum number of risk factors: 0

• maximum number of risk factors: 3

• The risk of acquiring a carbapenem-resistant Klebsiella pneumoniae increases with the number of risk factors.

• Acqusition of carbapenem-resistant Klebsiella pneumoniae is associated with an increased risk of death.
