Description

Escudie et al identified a number of factors associated with the need for liver transplantation following Amanita phalloides poisoning. These can help to separate those patients who may require transplantation from those who will not. The authors are from Hospital Beaujon in Clichy and Hospital Purpan in Toulouse, France.


 

Factors associated with a need for liver transplantation:

(1) early onset (< 8 hours) of diarrhea, which is usually cholera-like

(2) prothrombin index < 10% or INR >= 6 (often not evident until 4 or more days after the ingestion)

 

where:

• The authors use the prothrombin index/INR criteria at 4 or more days. However, according to Figure 3 this may occur at day 3 but may take longer to appear.

 

Performance:

• The accuracy of diarrhea onset was 78%

• The accuracy of the prothrombin index was 100% (100% sensitive, 100% specific).
