Shadnia et al evaluated outcomes for patients being treated for severe acute organophosphate (OP) poisoning. The authors are from Loghman-Hakim Hospital Poison Center and Shaheed Beheshti Univerity in Iran.
Parameters:
(1) need for admission to the intensive care unit (ICU)
(2) quality of the care in the ICU
(3) SAPS II (simplified acute physiology score II) during the first 24 hours in the ICU
Optimum ICU care may include:
(1) ventilatory support
(2) gastric decontamination
(3) therapy with atropine and oximes

where:
• A medical admission and need for respiratory support alone gives a SAPS II of 12. This would mean that any patient placed on a ventilator within the first 24 hours would have a poor outcome.
