Description

Oliveira et al identified prognostic factors for a patient with posterior urethral valves (PUV) detected in utero. These can help to identify a patient who may benefit from more aggressive management. The authors are from Federal University of Minas Gerais in Brazil.

Outcome: chronic renal failure

 

Factors associated with adverse outcome:

(1) oligohydramnios (relative risk 11)

(2) need for ventilatory support (relative risk 6)

(3) initial serum urea (? urea nitrogen) >= 40 mg/dL (relative risk 4)

(4) bilateral vesicoureteral reflux (VUR, relative risk 6)

 

where:

• The authors state the unilateral VUR was protective but the relative risk is 0.92.

• An initial serum creatinine >= 0.8 mg/dL was also associated with risk. This might be a bit more reliable to track than urea.

• The timing of initial laboratory tests relative to relief of obstruction was not stated. Other studies have shown that evidence of renal damage after initial surgery is associated with progression.
