Description

Gupta et al identified risk factors for mortality following an open repair of a paravisceral abdominal aortic aneurysm. These can help to identify a patient who may be a candidate for endovascular repair with a fenestrated or branched stent graft. The authors are from Creighton University and the University of Nebraska.


 

A paravisceral aortic aneurysm may affect the origin of:

(1) one or both renal arteries

(2) a mesenteric artery

 

The 30-day mortality rate after open repair was 4.5%.

 

Risk factors for mortality following open repair:

(1) female gender

(2) peripheral arterial disease (PAD) requiring revasuclarization and/or amputation

(3) COPD

(4) prolonged anesthesia time (odds ratio 1.004 per minute; at 3 hours the odds ratio is 2.05)

 

The presence of one or more of these risk factors may justify an endovascular procedure.
