Description

Quelard et al identified preoperative factors that are predictive of a prolonged range of motion (ROM) deficit following surgical reconstruction of the anterior cruciate ligament (ACL). The authors are from Centre Hospitalier Public d'Hauteville, Centre Orthopedique Santy and DDASS d'Haute-Savoie in France.

Criteria for a prolonged range of motion deficit - one or both of the following:

(1) deficit in full extension >= 5° and/or deficit in flexion > 30° at 3 weeks

(2) any deficit in full extension and/or deficit in flexion > 15° at 3 months

Risk factors associated with a ROM deficit:

(1) limited preoperative range of motion

(2) typical bone contusion (bruise) of the lateral compartment (lateral femoral condyle and laterial tibial plateau) seen on MRI

Additional factors associated with a ROM deficit:

(1) female gender

(2) operative delay less than 45 days

(3) any bone contusion (irrespective of location)

The need for arthrolysis (surgery to break up adhesions in the joint) was associated with bone contusion and a prolonged range of motion deficit.
