Description

Beard et al developed a score for evaluating a pelvic venogram in a woman with chronic pelvic pain. Today the testing could be done with ultrasonography, perhaps using microbubbles instead of contrast material. The authors are from St. Mary's Hospital and University College in London.

Parameters:

(1) maximum diameter of ovarian vein in mm

(2) time for contrast material to disappear after injected, in seconds

(3) congestion of the ovarian plexus

Parameter

Finding

Points

maximum diameter of the ovarian vein

<5 mm

1

5 to 8 mm

2

> 8 mm

3

time for contrast material to disappear

within a few seconds

1

slower than normal (20 seconds)

2

delayed (40 seconds or more)

3

congestion of the ovarian plexus

normal

1

moderate

2

marked, extensive

3

where:

• Some patients had contrast material persisting for up to 5 minutes.

total score =

= SUM(points for all 3 parameters)

Interpretation:

• minimum score: 3 (normal)

• maximum score: 9

• The higher the score the more marked the pelvic venous congestion.

• A score >= 5 was associated with the pelvic pain syndrome.

Performance:

• A score >= 5 had a sensitivity of 91% and specificity of 89%.
