Description

Urbina et al listed potential drug-related problems for hospitalized patients. The authors are from Hospital Universitari del Mar in Barcelona.


 

Drug selection errors:

(1) drug contraindicated or otherwise inappropriate

(2) drug not indicated (no reason to give)

(3) unnecessary duplication of therapeutic group or active ingredient

(4) drug not available in hospital formulary

(5) appropriate drug not given (indication not noticed)

(6) more cost-effective drug available

 

Inappropriate regimen:

(1) inappropriate drug combination (drug interaction)

(2) drug and food interaction

(3) inappropriate drug form

(4) inappropriate dose (too high or too low)

(5) inappropriate dosage frequency/intervals (too frequent or not frequent enough)

(6) inappropriate duration of therapy (too short or too long)

(7) inappropriate timing of administration

(8) inappropriate or suboptimal mode of administration

(9) inappropriate or suboptimal route of administration

 

Failures:

(1) failure to adjust dose for pharmacokinetic problem

(2) failure to monitor drug levels when appropriate

(3) failure to monitor for adverse effects (not in original list)

(4) failure to adjust therapy for deterioration or improvement in disease state (failure to monitor for response)

(5) failure in the use of a computerized order entry system
