Description

Ghaemi et al listed criteria for the diagnosis of the bipolar spectrum disorder. Affected patients have major depression and features of bipolar disorder without spontaneous manic or hypomanic episodes and so do not meet criteria for Type I or II bipolar disorder. The authors are from Harvard Medical School and George Washington University.


 

Criteria:

(1) >= 1 major depressive episodes

(2) no spontaneous hypomanic episodes

(3) no spontaneous manic episodes

(4) presence of findings in Group 1 and 2 matching one of the following patterns

Group 1:

(1) one or more first degree relatives with bipolar disorder

(2) antidepressant-induced hypomania or mania

 

Group 2:

(1) hyperthymic personality (with nondepressed state at baseline)

(2) recurrent (> 3) episodes of major depression

(3) brief major depressive episodes (averaging < 3 per month)

(4) atypical depressive symptoms (based on DSM-IV criteria)

(5) major depressive episode with psychosis

(6) first major depressive episode when < 25 years old

(7) postpartum depression

(8) antidepressant "wear-off" (respond acutely to antidepressants but these do not work for prophylaxis)

(9) lack of response to multiple (>= 3) antidepressant treatment trials at adequate doses

 

where:

• Hyperthymic personality indicates excessive emotion.

• I have not limited postpartum depression to females in the implementation since fathers can experience depression after the birth of a child.
