Parameters:
(1) current estimated GFR (eGFR) in mL per min per 1.73 square meter
(2) age in years
(3) urine protein excretion (albumin to creatinine ratio in mg/g, protein to creatinine ratio in mg/g)
(4) delta eGFR over past 5 years
