Patient selection: maintenance hemodialysis

(1) serum albumin in g/dL
(2) body mass index (BMI) in kg per square meter (using postdialysis body weight)
(3) predialysisserum creatinine to body surface area ratio (µmol/L per square meter)
(4) normalized protein nitrogen appearance (nPNA) in g per kg per day (a measure of protein intake)
